Am I missing something?

I have gone to the information seminar for the clinic I want to use for WLS.  They checked with my insurance, Empire BC/BS, and the requirements are a BMI over 40, pysch consult, NUT consult, and active participation in non-surgical methods of weight reduction.  The last part is a little unclear to me.  The policy reads "these efforts must be fully appraised by the physician requesting authorization for surgery."  Are they simply looking for me to provide documentation of weight loss efforts with the surgeon verifying that these attempts were legimate?  It also does not mention any specific time frame and the clinic said to just gather as much as possible.  If the insurance wanted a specific time frame or a supervised diet, would it state that in a more obvious way in the medical policy? Thank you for any help.  I have my initial consult in June but I'm trying to prepare myself now for any disappointment with the initial submission for approval.  The way I read the policy, it just seems too easy compared to what others have gone through. In my opinion you have nothing to worry about they are just looking for effort in the past it would be much more specific if they were looking for any more then that. So good luck but it sounds like you have a good policy so dont worry to much.
