5 year history

Okay, so I'm required to have a 5 year history or morbid obesity as supported by the medical record.  How do I prove that if I didn't have insurance during part of that time?  I was a grad student from 2002-2004 and I basically went to convenient care clinics for a cold here and there.  I'm sure my weight was documented when I went, but is that enough?  And what if there are no records? 

The care clinics should have your records. They put them in storage after a certain period of time, but i believe they are required to keep them for 7 years. I would call the ones you went to. lol jen
