Supervised questiom...

When an insurance company request records of a supervised diet history, what exactly are they looking for; signs of success and failure or signs of out right failure...?????

I appreciate you all responding so quicklt.  Falsifying medical record is definantly not the reason why I asked the question...me  would go know if that far becasue we both have too much to loose.  The reason I was   wanting know is because, i have about a year and a half of supervised  dieting records from my former employers physician and not my primary care physicain.  These record were partial the basis for my retirement in October.  I was asking because i did know if the records had to come from your primary care physician or any other physician you may have seen.

Supervised diets mean your doctor discusses a low calorie diet with you, and weighs you in each month.  To be safe, make sure your doc puts in your chart each month that you weighed in, and you both discussed your diet and exercise.  Some plans have become more pikcy than others where a simple report from a doc would suffice for one policy, others want to see detailed chart notes.
