To whom it may concern:
This is to request a reconsideration for the service of Laparoscopic Gastric Bypass Surgery. The letter I received from Humana Military Health Care Services states that I do not meet the criteria in the policy.
An quote from this letter reads, “the patient is 100 pounds over the ideal body weight but has no co-morbidities. Therefore your request for the Laparoscopic Gastric Bypass Surgery is denied.”
According to the policy only one co-morbidity is necessary for this procedure to be approved. I have been diagnosed with hypertension and Diabetes type II. I am currently taking Quinapril and Metformin in addition to diet and exercise in order to help control these conditions. Diabetes and Hypertension are both listed in the policy as possible co-morbidities, therefore I do meet the criteria outlined in the policy.
I have attached the prescriptions for these medications as proof of my diagnosis and treatment. Thank you for your attention to this very important matter.
