Question about 5% weight loss

My insurance, MCARE, requires 12 monthly visits to my doctor regarding diet and a weight loss of 5% of my excess body weight.  That is where I get confused.  When I started out I weighed 260 now I weigh 275 and I have 3 months to go.  I called the insurance and they say I have to lose 5% of  the difference from my current weight of 275 and ideal weight 155  which is 6 pounds but I think it should be more.  I don't want it to be more but it doesn't make sense to me that I can gain 15 pounds and still need to lose the same amount of weight.   Maybe I'm nuts and I should just be glad it is only 6 pounds but I don't want to be "surprised" in 3 months with a denial.   Thanks, Confused in Michigan Susan

Unless my math is bad  I think the 6 pounds is the 5% of the difference between 275 and 155.  IE - 275 - 155 = 120     . . . . .    120 * 5% (.05) = 6
