Opinion on Dr's Letter

Hi all I know I have read how important it is for your doctor to use the right wording in your letter I was just wandering if someone would let me know if this will pass............ I am a bit scared over the first sentance since it says: TO WHOM IT MAY CONCERN: I am writing this letter at the request of our patient, Mrs Kenna Mifflin, who is being considered for gastric bypass surgery.  She has a history of morbid obesity with a current weight of 242 pounds and 63 inches with a body mass index of 43.  She also has comorbidities of obstructive sleep apnea and gastroesophageal reflux disease. She has in the past used prescription phenteremine.  She has also been to weigh****chers.  She has used a a low fat diet with exercise and slim fast and has been unable to lose a significant amount of weight.  We have recently checked a thyroid stimulating hormone which was normal at 1.27 and that was done on 2-23-1997.  Her past medical history includes gastroesophageal disease, obstructive sleep apnea, and chronic bilateral lower extremity edema.  She aslo has a history of depression successfully treated with lexapro.  Her surgical history includes a cholecystectomy and surgery for an ectopic pregnancy and a tubal ligation. I think Mrs. Kenna Mifflin is very compliant in her medical treatment with us.  I think she is a good candidate for gastric bypass surgery.  Please feel free to call us with any questions. Does your insurance require supervised weight loss attempts? If so you will need your weights from your dr for each month written in your letter. Mine required 6 months. Otherwise the letter sounds fine.
