Hi!  My policy has the same 6mth pre-cert diet reqiurement I and would love to have the forms that show the physician what I need in my medical records.

I tried that link, but it gave me an error saying the link was outdated due to the changes in the CIGNA website. Aside from the fact that CIGNA lost my paperwork a few times, they approved me for VSG. They've approved some coworkers for RNY and lapbands also. You have their plan information thanks to Paul. Just be careful about the 6-month required diet. It must be documented for each individual month separately with date, weight, diet description, exercise, Blood Pressure, Pulse. You should be fine having just a copay for those visits as long as your regular doctor remembers to mention a comorbidity such as high blood pressure, sleep apnea, etc. each month.
