5 year weight history

What kind of paperwork do you submit to the insurance company for the 5 year weight history?  For instance, if I went to the doctor 3 years ago for a checkup, does the doctor need to make a copy of my chart for that visit (which has other personal medical info on it), or do they just type up a letter saying I came in on xx date and weighed xx?

they just need a date and weight. it can be in letter format.     (Your name) has been under my care and on (fill in date) weighed xxxlbs. sincerley  Dr. xxxx thats what i did and i got approved Adam

cigna is requiring me to submit actual medical records. I had a letter that was written by my pcp stating my long term weight issue. It didn't actually have my specific weights and dates on it. But when I got denied they said a letter would not be enough , they needed actual medical records. I didn't even think about the other info until I picked up my records this morning... it had all sorts of info that has nothing to do with WLS, but it did have my weight and date on each page... I paused for a moment but faxed it all over anyway... I figured if that is what it takes, then so be it.
