I have a copy of the form that my doc signed for me for my 6 month medically supervised diet.  A fellow OH'er sent it to me, b/c it worked for her, and now, I am passing it on, cause it ALSO worked for me.  If you need it, let me know, and I will gladly send you a copy.   (*I do not KNOW for sure if this will work for you and your insurance companies requirements, but so far, it's worked for 2 of us*) Best of luck to you all~ 

Was hoping you could help me, u mentioned a documentation form, can u PLEASE send me a copy???  My Dr and Cardiologist are both working hard to get me this surgery, but it is not coverd by Medicaid here in TX, but was hoping that if they had all the facts it may speed things up.  Or maybe change things. I am 42 y/o, am diabetic, high blood pressure, angina, with coronary artery diease, u'd think that would be enuff to get help, but not sure it is.  But any ways maybe with the form it will at least speed things up(MAYBE).
