UHC appeal on plastics...

Hello! I need help!!! I have submitted a letter of pre-determination to United HealthCare for removal of skin from my stomach and arms. My pre-determination was denied and I have already submitted my first appeal. Dr. Marisa Lawrence's office was not very helpful in directing me on the appeal. Natalie stated to just "write a letter and tell them how it affects my mobility, hygeine, etc." I went ahead on my own and got a recommendation letter from my chiropractor and sent that and some pictures with my appeal package. Now, after reading posts on this website, I am scared and confused that my appeal will also be denied because I don't have any documentation from a dermatologist. I have the following questions: Has anyone been approved for plastic surgery with United HealthCare? If so, what documentation did you provide? Are there any plastic surgeons in Atlanta that are good with directing you on the appeals process? How many appeal opportunites do you get? If I am only allowed 2 chances, I want to make sure that I have everything in order before I submit again. I have lost 160 pounds and my stomach hangs in every direction. I have also submitted a complaint to the Insurance Commissioner and have copied UHC on the complaint in hopes that they will know that I am not going to go away easily. Any advise will be greatly appreciated. I did make sure that the codes used were "reconstructive friendly" and the pre-determination did ask for pannilectomy (sp?) and brachioplasty (sp?). You want to look at the policy and see how they define reconstructive plastic surgery and then make sure your medicals fit into that definition.  I/E if reconstructive is warrented because you have trouble walking etc.. then you'd make sure you have the records to show that as a problem if it is.  We get them approved only with really good documentation.
