Are all company policies the same, with the exception of employer exclusions?

I have UHC - have not received my 2007 policy, but will it be the same policy as Miami Teachers policy, with the exception of employer exclusions? If so, I'm guessing coverage is not possible. I'm 36 BMI, and have at least one comorbidity (high blood pressure), possible 2 (sleep apnea) - however, the ryder says that treatment failed to cure the comorbidity.

UHC loves exclusions and they get away with it in states like florida.  Chances are if the employer has a rider added they may add it again.  Can you work with your union to get them to include a rider?
