Am I covered?

This is my certificate of coverage for UHC.  If you look at page 124 you will see the ryder regarding WLS. Does this mean I have hope being covered?  If so, is there anything I have to becareful with when submitting my request? http://riskmanagement.dadeschools.net/benefits/2007/coc/uhc- POS.pdf

Looks good.  You'll just have to keep an eye on a few things. 1. Has to be a member doctor 2. designated facility 3 not experimental. So, just make sure your surgeon is in network, the facility is in network and approved, and the they DO NOT consider the surgery you seek experimental. Make sure your surgeon submits a full diet history as well.

One more thing:  I've been under my primary physician's care for weight loss for about 4 years.  Will this count as their requirement or do I have to have an additional year.   The rider also states I need to submit a second surgical opinion.  What exactly does that mean?  Is it going to another surgeon? Thanks a million
