PCP - Letter of Support Question

Hi - My PCP is not familiar with WLS and she is leary of what she needs to provide. Basically, she wants information from the surgeon on what she need to give. She doesn't know what the letter is supposed to entail. I have BCBS - PPO and was told that the letter just needs to include: 1. how long I have been in her care. 2. what different diets I have tried and failed. 3. any weight related medical conditions. 4 "medical necissity" How can I make this easier for her and myself. I have only been in her care for a month or so and don't have any co-morbidities (thank God) yet. What should I do? Write the letter and have her put it on her letter head? Does any one have any suggestions on this. Thanks - Steph

honestly i took one of this site and my dr typed it up on her letter head simple as that if you dont have comorbidities get one or two i.e. joint pain you treat with aleve, back pain, edema your ankles swell when sitting for long periods of time, skin rashes-- come up with something just cuz you dont take medicine for it doesnt mean you dont have a co morbidity-- good luck to you!

I am new to this site and was curious as to where you got the letter? I have been trying to find one to use to send to my insurance company. This is the last thing that I need to do before I submit to insurance for the first time. Any help would be greatly appreciated.

Steph, Melissa is right, I also came up with a letter. My surgeon said I needed to have the things you mentioned and then I was denied because insurance said it was not specific enough. From this site and a site on yahoo call bandstersinsurance I got letters that other people had written and read them and took what was true for me and added what was missing. I then went to my pcp and gave him the letter and told him that this is what insurance needed he had it put on his stationery and signed it. Since your pcp is asking for help do the letter also see if your surgeons office has a letter from a patient with your insurance that was approved and she if they can give you a copy with all of that patients personal info(name, address, etc) blacked out, if they can do it it would be helpful because that person was approved. For me there was no one with my insurance approved so I could not get a letter.  It really needs to be detailed that was what my insurance was looking for. Good Luck
