Insurance coverage of revisions for self-payers

I have insurance that covers the lap-band surgery, but it requires a 5 year history of BMI > 40, which I do not have.  If I self-pay, then have later complications, am I stuck paying for that also? Does the insurance cover that since it's now a medical problem?

Its my understanding that if you do not qualify for surgery using their guidelines and they do not approve your surgery, then any complications that arose from surgery would not be covered.
