LETTER FROM PCP

Just wondering what information the letter from my PCP to surgeon should include.  I have a physical tomorrow, as I was denied because I didn't have a letter from my PCP.

Thanks so much.  I have the letter drafted and going to take it with me to the PCP today and if she is good with it, going to have her print it on her letterhead, I think that is called by optomistic or is it proactive.  Maybe a little of both.  Just trying to satisfy CIGNA -- Thanks again.
