my daughter has UHC-Choice Plus Plan is waiting for approval for lapband.  From what I have heard on this board they approve quickly with no questions.  Check older posts everyone seems to like it.  We will see, her paperwork has been in for 7 days.  One lady on the board heard back in 10 days and got approval.  Good Luck

I have UHC Choice Plus and it's a complete exclusion.  You need to read your insurance docs.

Hi, I have UHC PPO and they approved my WLS after an appeal. It all depends on whether your employer bought a WLS excluded insurance package through UHC or not. With mine, it was covered, but I had to show 5 years of weigh-in's at my doctor’s office (one per year) and they all had to be with a BMI of over 35. I, also, had to do a 12 month doctor supervised diet and a 5 year diet history along with all the things the surgeon required. I saw someone else who got approval with absolutely no problems and no requirements other than BMI of 35 and she also had UHC PPO. Her employer had no exclusions for WLS and no WLS requirements, so it was super easy for her to get approval. That is the way it should be for all of us.  Good Luck,

1st of all, thanks to everyone for giving me the heads up on this topic....... UPDATE:   I have called my insurance company and they told me that there was no pre-requisites, only ONE year of weight history, but I don't have to follow a supervised diet like most insurances require... UHC will cover 90% of the surgery after a 300 dll deductible... I was told by the billing lady in dr sweeny's office that the 10% I have to pay will be less than 1,000..... because all the exams, phychologist, nutritionist, sleep doc, etc.... will be covered 100% (with small 10 or 20 dll copays).... Before she knew that I had that insurance, in the Info session she had mentioned that UHC was by far the easiest to get approved...  Horray ~Dee
