GHI/HMO NY

I wrote this on the NY site also, but figured I might get more answers here.  I have GHI/HMO NY, I called them a month ago and asked what is required to be approved for GS...they said I needed 12 months physician supervised diet...and I asked them to send me a copy of the medical coverage policy on surgical treatment of obesity...I just received it friday..and with what I'm reading, it looks like to me that I only need 6 months physician supervised diet...."recent (within the last 12 months) history and documentation submitted as evidence of active participation and reasonable compliance with at least 1 physician-directed weight loss program for a minimum of 6 months."  does anyone here have any experience with GHI/HMO..and do you know what you had to do to be approved...Because what I think I'm reading, it's supposed to be 6 months...any help would be appreciated..thanks
