Out of pocket costs besides what insurance covers

I was wondering how much did everyone have to pay for things such as: -Psych Eval -Pre-Operative History & Physical -Upper GI X-ray Series -12 Lead EKG -Chest X-ray: PA & Lateral My insurance company is HMSA (Hawaii) I'm not sure if they only cover the cost of the surgery or if they cover the costs listed above. Well please write back!    

Have you contacted your insurance to verify that they cover WLS?  If so, I would call them and find out if they will pay for the pre-op testing. 

Hey thanks for the response! Yeah I found out that they do cover the costs for the tests, and that all I'd have to pay is a payment to the surgeon for follow-up visits for the next few years.
