I forgot to mention..the above i just a guideline for Anthem.  Each person's plan will vary based on the the clients (employers) preference.  This info just happens to be the same as what was given to me by Anthem when I call based on my plan but yours may vary.

Thanks!  maybe i can get some answers there, iam trying to find out a lot before i make my appt to my surgeon,  but with this exclusion i dont know what to expect.

This is what I was told everytime I called as well.  I have gone to 2 consultations where I was told Anthem required 6-18 mo medically supervised dietting.  So we will see!!
