Letter needed to submit for Plastic Surgery

I went to see the Plastic surgeon on Friday about a Panniculectomy and Brest Lift. I ‘m waiting for my last 4 months of documentation from my doctor office about the rashes that I have on going. My doctor also called me and stated that we will write me a letter to submit. I just need help to write a personal letter to submit to the insurance company with all the requested information to support my claim. Is there a letter I could use to get me started or a good idea?  

I don't have any experience writing a plastics letter as you've requested.  However, I do a lot of mentoring for new business owners and get this question a lot .... What do I write? (sales letters, websites, brochures, etc.).   My best advice:  Just start writing! Write down everything you want to say at first.  Don't edit yourself or try to make this first draft perfect, just get your thoughts down on paper (or computer screen).  Then when you can't think of anything else to write go back and edit, edit, edit.  Have others read it, then edit some more.  Set it aside for a day or two, then edit some more.  Eventually you'll come up with something you're happy with if you just go through the process.   Let us know how you do!
