does anyone know the req. for this ins.? Is there a 6 month diet? or any testing that they req. before an aproval?

You need to get a copy of your benefits booklet and look for specific information on this type of surgery.  Most booklets now list the criteria needed.  If it doesn't, you should request it from them prior to looking for an approval. Victor
