Humana - Plastic Surgery Coverage

Can someone tell me how Humana PFFS is regarding having a TT and BL/BR??  I had switched from Health Alliance Plan (aka HAP).  I have not been able to find out anything in the book they sent or when I call them.

I had BC, but I know with plastics, which I had, they want to see that it is reconstructive surgery and not cosmetic so you have to show rashes and things like that.

Of course, it is best to always check one's explanation of coverage for the final word on coverage.  However, as a general rule, plastic surgery is not going to be a covered benefit.  The question that an insurance company is going to ask when it gets a request for a service is, "Is this service medically necessary?"  If you answer "no" to that question, then you can basically count on the same answer from the health insurance company.
