Policy bulletin for UHC

Does anyone know how to get the policy bulletin for obesity surgery off the United Healthcare web page?  I am trying to help a friend do this.  I was able to find the one for Aetna but am not having luck with UHC. Thanks

I do not think there is one available as on Aetna.  I have even called UHC and requested the requirments for Surgery and they told me that they would only release that info to my Dr. (what a crcok!) Chris
