Needing insurance letter examples

Hello, I'm needing help from anybody who has had to write their insurance company for approval for plastic surgery.  I am 2 years almost 3 years Post-op come Feb. 2007.  I have went and seen my plastic surgeon Sept. 11, 2006.  All I need now is to write the insurance company for approval.  I really don't know how to go about doing this, what I should say in the letter ect.  Any help from others would be terrific.
