Cingular Wireless

I have tried to obtain a copy of my Summary Plan Description from both the insurance company and HR... and they keep giving me the run around.  I currently have United HealthCare Choice Plus.  When I inquired about surgery, all I was told was my insurance didn't cover it.  Does anyone have any suggestions on how I go about getting a copy of the SPD and where I begin to see about my insurance possibly covering the surgery?

Make a written request to your HR department and send it certified mail. I am an HR Director ;)
