I have Empire BCBS PPO, I live in NC, work in SC. I needs the 411

I am having issues with the 6 month documented behavior modification thing as well. I don't have 6 months straight of documented info. But I do have appointments setup for the next 4 months with my pcp. But I saw her in June and we discussed a work out plan. I hope and pray when I see her next week she will updat e and document reasons as to why I couldn't get in her office for the last 2 months. I tried though.

you should be ok.   The doc can convert it to a report that theyve seen you over the past 6 months etc. with little or no success.  That you've counseled on diet etc...
