What if I am familiar with my insurance co, and they are telling my Dr's office something they're not telling me.  I've been advised that I need to supply a 5 year weight history detailing my morbid obesity.  I haven't been morbidly obese for the last 5 years, so I can't provide that information.  Now my Dr's office is slow to get back to me, because they know I can't provide this requirment, but  I know other people have been approved without this requirement......HELP!!! Alli

My insurance booklet says: Surgical treatment of morbid obesity may be a covered benefit when:

It is determined to be Medically Necessary; and

It satisfies the criteria established in the Claims Administrator’s medical policy guidelines But I am not sure how to find out what the "Claims Administrator’s medical policy guidelines" are.  How do you find that out?  How do you find out if they will cover lap bands too? Much thanks
