Daily Post--2/9/08
I know that I have created a weight tracking chart and body measurements chart that are both on my profile in the "my story" portion. I also use a spreadsheet to track the weight slips from my surgeon's office (body fat, fat free mass, body water, etc) as well as my full list of body measurements (more complete than what is on my profile chart). I also have a couple of spreadsheets I use--one to track my complete list of lab values AND one to do daily food logs. I can share both of those if someone was interested and emailed me at [email protected] (not a PM here as you can't attach anything!).
I don't really use any other charts or tables but am curious to see what everyone else uses!

I haven't used any charts--at least not since the very early weeks/months!
At first, I just used the very simple chart that was included in the materials from my bariatric center. It was just a sheet w/ columns for Time of Day, Food Eaten & Amount, Protein Grams, Calories--and then a question at the bottom for Vitamins Taken? and Ounces of Fluid. I probably used that for the first couple mo. or so.
Otherwise, I just cram anything like lab results, interesting wls articles, etc., in a big binder I have labeled "WLS"!
I'll be checking back later to see what other "old timers" like me are using.
