what tracker do you use?

so many people have been talking about food diaries, etc., i wanted to know if there was one that your doc gave you -- or did you make your own, or where do we get these things?

Height: 5'6".  Top Weight/Surgery Weight: 274. (August 2010)
8 weeks out:  219.6.  12 weeks out:  198. 
NORMAL WEIGHT REACHED MARCH 2011 (153 lbs.).
Goal BMI:  In the normal range, please -- somewhere between 22-24 
Clothing size 26/28 (triple X) before surgery in August of 2010. 
March 2011 clothing size:  10/12 (large)

I use the one on ObesityHelp. If you go to your start page , go down to Health Tracker. It has a place for you to keep track of weight, measurements, food & exercise.
I use it faithfully.

My doctor gave me blank pages to make a food journal - it included what you ate, how much, when you took vitamins, and how much exercise did you do. I just made a bunch of copies and put them in a folder.

The Daily Plate (online) has a good one. They have a really large library of foods so you don't have to manually enter very many items.
