appeal letter

Hi looking for some help on an appeal letter.Has anyone wrote one to the insurance company and then they were approved?I'm not sure the format or exactly all that I should put in.I do not want to sound unprofessional but I want it be from the heart with medically correct info.

Your surgeon should be able to help you as well.

 I thought your doc's office did the appealing for you? Could definitely be wrong though.
