Good luck! Just make sure you include all documentation you have.  I know that primary care physician office visit notes are very important. Make sure your physician noted the things that your insurance requires to be included. My surgeon's office helped me with these.  -- Tom

EVERYTHING is DETAILED.. I have INFO OVER KILL. Documentation from the dietitian, the PCP, my orthopedist, I have 8 years medical records showing my weight, I have Every receipt as well as my weight record from Weigh****chers, I even have a LETTER from the Weigh****chers coach (VERY NICE LETTER BY THE WAY). I EVEN got documentation that shows my family medical history. The only thing that I am waiting on is the Psyc rvw that will go in Oct 24. and  they have me going to see a NUTRITIONIST (Not the dietitian) for an eval on 11/1. Once that is complete they will send the pack to the insr company. So Im almost there. I mean work will prob make me wait till Jan to actually do the surgery. But, they say in a few days I should have an answer and be able to schedule at date. I will prob be celebrating the new year on liquid.. But, I can make the sacrifice.  I guess 6 mo goes FAST..

Good luck! I think Jan is a great month for a re-birthday. But then again, I am a little biased. Look on the bright side, if you have a max out of pocket for your insurance it gets taken care of at the beginning of the year. I have had 3 upper g.i's, another surgery, multiple trips to the E.R. (I had a dehydration problem for a while.),  and I am getting ready to have a scar revision on an ultra sensitive scar and it's all been free.
