Question about PCP letter...

Hi My PCP is writing a letter to Aetna endorsing the surgery, and asked me if there was anything in specific he needed to write (any particular phrases, etc.) I'm not really sure either! Do any of you guys remember what was written for you that worked? He's going to basically say what's wrong with me physically and then give RNY the big thumbs up, but is there anything else? Valarie

Valarie, i would ask him to include all of your weight for the last three years, as well as all the supervised diets that you have been on... success' and failures! he needs to let it be known that he feels you would be a good candidate for the surgery. good luck to you! I have not had the psych, but I'm going to set it up today with my regular shrink- she's fabulous, and she knows me

Hey Valerie, my surgeon gave me a "sample" letter to give to my pcp. maybe you can ask if they have one, what surgeon you using?

I'm using JK Champion in Marietta, so I won't actually see him in person until I am either pre-authorized by Aetna, or I agree to self-pay. Do you still have the sample? Valarie

Valarie First I have to tell you that you have an AWESOME name and your mother knew how to spell it RIGHT!!!! I will go into pmy archives at work tomorrow, because I think I still have the letter that my PCP sent. I basically wrote it for him and he signed it, but it was just a very extensive letter regarding all my health concerns, diet history, things that weight loss would help and quality of life issues.
