i need MAJOR help....

can someone please help me with a sample diet history? i have been struggling with this deal for hours and i am at my wits end.... my "clinic" is scheduled for tuesday morning and i am worried that i will have to reschedule since i cant seem to get this completed. i know i may be stressing about "nothing" - but i really suck at doing things like this. i am sooo worried i will be denied cause i cant remember ever fricken diet, time frame, amount lost - urrrrggg jb please help
