Medical Clearance Letter

Does anyone have a copy of a medical clearance letter they would like to share? Not a medical necessity letter, but the letter your PCP or other Dr writes saying that you are cleared for surgery. My PCP wrote one, but I'm not sure it's correct and I don't want to mess anything up with me having surgery on July 8th. If anyone does, please email it to me @ [email protected] Thanks

Hi, I am not sure exactly which letter you are referring to. I have BCBS and I had to have a letter from my PCP which stated specific wording that my insurance company required at the beginning of the approval process. It had to state tthat I was medically cleared for surgery or low-risk for WLS. I am about to have my pre-op work-up done and enclosed in my admissions paperwork is a letter from my WLS doctor (not my PCP) which outlines details from my file including his recommendation for me to have the surgery also. So, there are two letters. One from my PCP which went to the insurance company with all my paperwork prior to approval and a second letter which is included in my hospital orders that I bring to my pre-op appointment at the hospital. In otherwards, one letter from PCP, another letter from WLS doctor. Hope this helps. Sincerely, LMW
