Info on BCBS of GA HMO

Hello there, if you look on my profile you will see a list of all the things I had to have for my surgery also and it's the same thing they are saying to you. Looks like what they are requesting hasn't changed for the past few years. This is all I had to have. I had to have a 5 yrs wieght and diet history as well. I also needed a letter of medical necessity that was seperate from the letter of clearence. Dont let them stump you on that. One says why you need it the other says your medically cleared to have the surgery. Dont give em a excuse to deny you. Give over and beyond they can be shady.... NAT
