kaiser advice...

is there anyone in here who has had the surgery through kaiser permanente ga? or even better who works for the home depot?if so...what were their requirements?how long did it take to get approval?and do you have any sample documents(medical necessity, diet history) that you could e-mail me? i would just like a format to reference...thanks for any feed back
