12mth diet history/ Dr. Cox patients?

This is for anyone useing or has used Dr. Cox. I was wondering how your having the diet history submitted. Is your PCP includeing it as part of your medical necessity letter or is the PCP putting something different together? I was wondering how I should do this? Should, I have him include it in the letter or should he put together something else? Any adive would be appreciated. I had included this in my letter, the one I wrote for my PCP. Hmm, help! It's best if he includes your 12 months diet history with the dates and your monthly weights listed in his letter of medical necessity. Mine, didn't write it that way, but Carey said that would be the best way to do it. Hugs, Erika L
