Need help w/PCP letters, can anyone help?

I need your help everyone. I have to create a letter for my PCP to sign. One that can be sent along with my request for surgery. Can anyone help? I know many of us have to create letters for PCP's to sign. I had to do this for my former PCP and OBGYN as they are so busy it was taking months for them to even get around to thinking about it. Can any of you e-mail me the letters you used from your PCP so that I can compile one for mine? I know all of our journeys are not the same, and sometimes the only thing we have in commom is the fact that we are obese. But I so desperatly don't want to be denied again. (last deniel came because blood work was not completed & submitted with request for surgery). I want to be the prepared paitent when I go for my appointment with Dr. Thoburns office. I realize the impact a PCP letter can make on BC/BSFL H.Options/HMO and I want to be prepared if I'm denied for the appeal process. Can you PLEASE help me? My e-mail address is [email protected]. My suggestion is to write a letter that deals with the physical/emotional/vocational/social problems that you are experiencing as a result of your weight. Include such things (if they are applicable) as stress incontinence, lack of mobility, breathing difficulties, gallbladder problems, sadness, depression, lack of a social life, etc. ALSO, include any and all attempts at losing weight in the past, name programs, the amount lost and the following weight gain...I think these things will help them understand the medical necessity for the surgery. So what your saying is write my own letter don't get a letter from my PCP? I thought I was to do both? Are you saying that isn't so. I was under the understanding that I needed a referral letter from my PCP in addition to my own personal struggle letter. I'm confused now. Thanks Marilyn

Marilyn, So sorry to confuse you. That was info for you to write to insurance company. The PCP needs to cover you history of weight loss and gain, your BMI and why he believes you need WLS. Also any medical conditions as a result of your weight. Marla

you should go to gastric bypass family thats where i found a med ness. letter that has everything in it.

Hi Angela: Thanks, for writing me back. You mentioned going to gastric bypass family. What is that? Is it something that is here on WLS site? I have begun doing a letter but don't know if I'm doing it write, to look @ something would give me a better idea on how to write it for the doctor.
