Question

I saw somewhere in a post here a link to a sample letter to write to the insurance company. Can anyone tell me where it is or in which post I saw it in, I can't find it now.

I used a letter I found on gastricbypass family.com. My PCP found it helpful in using it as a guide when writing their letter.
