Letters To Insurance Company

I am working through Dr. Overcash and I was wondering if anyone had copies of letters from themselves to the insurance company that they might want to share with me. I am very concerned about getting approved through BC/BS Health Options. Also.. if anyone has insight on their process please help!!! Thanks.
