Has anyone with Premera Blue Cross been approved for the surgery?

You can check the database on this site of insurers by state. I've come to realize that it depends on your policy if you are covered. For example, my friend and I both have UnitedHealthCare, her policy has an exclusion on WLS while mine does not. I've called UHC to verify and was told it does cover WLS with a diagnosis of morbid obesity. My friend's policy specifically excludes any treatment for a diagnosis of morbid obesity. The difference is her employer purchased a different policy than my employer did, although both of them purchased policies from UHC.
