Blue Cross / Blue Shield XJB

Hello everyone, has any one had experience with BC/BS XJB insurance? my doctor doesnt want me to file the insurance until I have 5 years proof of my weight in doctors notes (i only have 4) they originally said 2 years. and now they want me to have 6 months under a doctors care specifically for weight loss... not to mention all the other things i had to turn in. anyone have any advise on dealing with BC/BS XJB?
