any know the requirment for this insurance, how long do you have to wait

It'd be best to just call them and ask what is required. As requirements change daily!
Open RNY 5/10/06 and now

Lap Band on 04/17/09
I went to see my doctor on a Monday.  He said "let's do this".  He sent a letter to BC/BS State of Florida and I was approved within two weeks.  Very painless.  Had my surgery on April 17.

I have bc/bs federal and my approval only took 1 day... I still had to see a NUT and a PSY but everything else was a breeze!
