Need to find a Dr. in Lakeland area for medical program

Hi, we are in Florida for the winter. I am having  Lap Band in Ct. next summer. I need to find a Dr. in the Lakeland area for a medically supervised diet and exercise program for my insurance company for 6 months. I have Unitedhealthcare.
Can anyone help me with this?

I would call your insruance company and ask them for a list of approved providers

Thank you! I did call my insurance company and they referred me to a Dr. in Lakeland. I see her on Friday.
