I am on Medicare and my surgery was done June 5, 2006.  Since we moved we didn't have a bariactric hospital in our area.  February 15, 2007 I had a perforated ulcer that blew the stomach pouch out.  The first part of the Roux en Y surgery had to be re-done.  They sent me by ambulance to the nearest hospital that had bariactric surgeons and the equipment to re-do the surgery.  Now Medicare won't pay for the ambulance to the second hospital.  I have appealed several times and now am up to an Administrative Judge.  Sent copies of hospital records, ambulance run sheets and letters from the doctors.  This most certainly was medically necessary .. as the doctor in the ER at the hospital here told my husband.  I could try and repair but I am not a bariactric surgeon and she could die.  So much for Medicare and the elderly.
