I'm in Canada as well, Ontario actually. Just curious about the order in which you are doing this. Did you first go to your Doctor, have him fill out the OCC forms, then YOU selected your surgeon or did the Doctor have him in mind ? Please, if you could share the details with me either on the board or send me a pm. I'm trying to start the process but having trouble getting people to layout the proper process. Any help would very much be appreciated.
