Dr. Lutrzykowski's recommended bloodwork

Good afternoon fellow DSers. I was just wondering if anyone would have a copy of the required bloodwork, as recommended by Dr. Lutrzykowski that they could send me. I have a new family doctor and I can find my list anywhere. I should really know it by heart after all this time...lol
