My Psychologist has trust issues
I wouldn't trust her either. Something fishy is going on. In addition to what Val recommended with contacting your credit card company, I would report her to your insurer. I don't know if it would help you financially, but they might take her off their list. I would also report her to the hospital that recommended her.
This is not appropriate conduct for a professional medical provider. You should have been informed up front of any and all charges that were your responsibility, and the charges should not have changed, nor should you have been billed for anything other than the charges that were discussed.
And what is the reason for her wanting to stop going through your insurance company? That's what your insurance is for. Sounds fishy to me.

Thank you everyone for your replies. This assured me that I wasn't in the wrong. No matter what, even if my insurance denies it officially, she took money that she should not have from a card I trusted her with. I left a message with the Bariatric Nurse on Friday to call me back. She normally calls me back within a day or two. I might go ahead and call the surgeon's nurse and let her know what's going on as well.
So, I called the Board of Psychology and asked them if I should submit a claim. They said that some of the things she's doing is unethical and I should. One thing they mentioned was the fact that she has refused to send the report without a promise of payment right away. They said she was not allowed to keep reports due to non payment. I don't have an issue paying what I owe, but the way she is doing things is off.
I submitted a complaint to the insurance company as well since she's trying to go around them and collect payment rather than going through them and waiting for results.
Thank you everyone for your advice. I did also contact my credit card company and explain she was not authorized to use my card any longer.
Update: So she resubmitted the claim and the new ones got approved. The co pay for me is $20.40. She's taken $235, so I'm going to go ahead and submit a dispute to the credit card company. She's overcharging the insurance company and not going with her contacted rates with them. She had me sign a waiver, but she's violating her own paperwork by charging before results come back. Even if I had to pay the difference, she still owes me a refund.
