I hope things straighten out for you. The only advice I can give you for fissures, hemorrhoids and pain is....I stared using a warm water and small amount of salt enema after each BM to cleans the rectum and wash the acidic poo away when I have acidic poo. I just wash the enema out and store.  After, I apply natural Aloe gel to my anus and push a little up inside as well to help with healing. The salt enema and the aloe have healed my one busted hemorrhoid and the heat from the warm enema has helped with the inflammation as well. I also started taking epsom salt baths once to twice a week to help with inflammation. The Benefiber has really helped quite a bit in decreasing my acidic type poos. Also, the increase in calcium has helped.

You might want to see a gastroenterologist if you haven't done so already.

From the symptoms you describe,  particularly if have had your gallbladder taken out with your DS, seems to me this could possibly be bile salt diarrhea / bile acid malabsorption? (http://en.wikipedia.org/wiki/Bile_acid_malabsorption ; http://www.gihealth.com/html/education/bilesaltDiarrhea.html ; http://bilesalts.net/diarrhea-dealing-with-the-unpleasant-side-effect-of-bile-salts/ )  I'm not a doctor, but I am familiar with similar symptoms, having personally had bile salt diarrhea since a cholecystectomy last yearand am pleased to report that it is very successfully treated with 3 Welchol (Colesevelam) twice daily with food.

I hope you find relief soon!

:) Hilary

ACtually I think you just diagnosed me. I have had this problem for years and it started after I had my gallbladder removed. I have always had IBS but mostly constipation. After my Gall Bladder was removed I started having acidic poos. I just researched and read about studies on this and the medications. Unfortunately these medications also keep you from absorbing vitamins and minerals and require you to be on a low fat diet. That is BAD for a DSer. The study did say that if you cannot take this medication that you can take Imodium several times a day as well as taking fiber supplements and that it works too without the mal absorption. I have the unfortunate luck of having this issue for life according to what I read. I have been on Imodium and Fiber since I posted this and it has made a huge difference in my life. It makes me feel better to understand what my issue is now. Thank you for the info.
