medical alert bracelets
That blind GI really applies to the RNY not the DS or sleeve. I just carry a card in my wallet that shows a picture of the DS configuration and has my surgeon's contact info on the back.

Okay, no blind gi is RNY info sounds like the nurse knows about THAT one...but we aren't RNY'ers.
I've had a medic alert necklace (DESPISE bracelets) since 1980...after I had my DS in 2011, I just ADDED the information to my list of medical conditions (I also REMOVED the information on my insulin pump :) ) But had I not had one already, I would not get it just for the DS. I WOULD keep that info available in your wallet (the diagram as well as contact info for your DS surgeon)or scan it into a cloud service like dropbox or box (both have apps for smart phones) and make it so you can access it offline.
